Thinking that they can save at least $100, Debra let's Ray play.
At the golf course, Ray begins to feel flush. After freaking out a little, he heads to see Dr. Sundram. Dr. Sundram examines him and tells Ray he had a minor anxiety attack and that he needs to do things that make him relax, such as playing golf.
So, Ray had an anxiety attack because he felt guilty about lying to Debra. And now, he has to go face her and tell her that the doctor prescribed more golf. But, the thing is, Ray didn't mention that he tricked her.
Lying to Debra once again, he feels even more guilty, and has to confront her with the truth.
